This release form is for anyone under the age of 18 who is planning on flying in the wind tunnel and will not have a legal gaurdian present. This form will need to be need to be notarized by an official notary for the form to be accepted.
Any flyers under the age of 18 who do not have a legal gaurdian present and do not have this form will not be allowed to fly.
This form is a general release form that you can view. You can either download this form now and bring it with you, (unsigned) or fill one out when you arrive at SkyVenture Arizona.
